what kind of doctor is he? looks more like stage makeup and prosthetics. if the little girl is happy, sure. let her be. however, something in her mother/female guardian’s eyes tells me the little girl is somehow required to pretty herself up. sadly, if that’s the case she’s in for a long, miserable haul. if not, and it’s genuine, then cheers to the little actress!
By the time you are approx. 5 or 6 ears old, your ear is nearly the size it will be your entire adult life. So, the likelihood that she will need another one is quite small.
My daughter was born with this particular birth defect, called Atresia/Microtia, and perhaps a quick Google search would go a long way toward shutting your trap.
